Enrollment & Forms

Enrollment for the 2024 -2025 year has begun!

Complete the 4-H Enrollment Packet and return to our office. This packet includes the 4-H Medical Release Form, 4-H Medicine Form (if applicable), 4-H Enrollment card, and the 4-H Code of Conduct Form.

There is an online 4-H Enrollment Form that can be accessed via the QR Code below or via this link.


Eligibility

Any person, regardless of race, color, national origin, gender, or disability may enroll as a 4-H member.

All school aged youth in Fayette County (K - 12th grade) may enroll in 4-H clubs and groups.

4-H does not charge dues or require uniforms. However, there are fees and dress associated with certain events and programs.
