4-H Required Forms

Lowndes 4-H requires certain forms to be kept on file for each child who wishes to participate. These must be renewed every school year before a child will be able to participate in any 4-H activities, virtual or otherwise. ?

Please be sure to complete the online enrollment form first. When your child participates in an activity, you will? complete the Medical Information Release Form and email the form to our office at?uge4185@uga.edu or print and drop off at 2102 E? Hill Ave, Valdosta GA 31601.

  • (For grades 4th-12th to sign up for 4-H)
  • (Should be completed 1-3 weeks prior to event or activity)
