Basic Forms?

4-H'ers must complete a new Enrollment Form and Code of Conduct each year.? This one copy will cover all events (virtual or in-person) that occur between August 1, 2023 - July 31, 2024. A new Medical Release Form and Medicine Form (if needed) are required for each event.

Virtual Program Forms

4-H'ers who particiapate in online or virtual programs must complete both the Electronic Consent and Participation Agreement/Waiver form. These forms will cover all online events that occur between August 1, 2023 - July 31, 2024.
