The following forms are required to participate in all 4-H Clubs or activities.?4-Hers must complete a new Enrollment Form and Code of Conduct each 4-H year (August 1^st - July 31^st).??

• Code of Conduct

A new Medical Release Form and Medicine Form (if needed) are required for each event.

• Medicine Form

• Medical Form

For alternative travel to or from a 4-H event and not with the county group.
